Fully Remote Tele-Health MAT Appointments Terms and Conditions
This Tele-Health MAT Appointments Terms and Conditions explains the details surrounding this type of service offering.
Enrollment in the Fully Remote appointment program is not a guarantee. Providers and staff have full discretion over eligibility and will be based on cooperation and/or behavior of each individual. Minimum requirements to schedule a full tele-health MAT visit are as follows:

Access to a device capable of running Zoom applications both audio and visual.

A Cell phone and Email address for contact and documentation signing.

Must be willing to perform self drug screens.

Must have the ability to take drug testing kit to a Fedex drop box / location to return it.

You Must be in the state of Pennsylvania during your tele-health appointment.

For Self Pay Patients and/or patients who have commercial insurance and would prefer to do a Urine Drug Screen instead of the oral fluid swab, you will need to come into the office once every 4 months. More detail can be explained at your visit.
Sublocade, Vivitrol, and Brixadi patients DO NOT qualify for fully remote visits as the injections must be given by a provider in person at one of our offices. However, Tele-visits for suboxone or REVIA for bridge scripts can be acceptable.
